The only pitfall is that Google must have indexed the address in order to have it show up, but that is logical. In case that happens people can still manually fill in all the address fields so no functionality lost because of that.

What do I need for this to work?
Easy Digital Downloads with the Recurring Payments extension. The survey hooks into the Cancel button customers already use, so there is nothing to add to your site.
Can I require an answer before they can cancel?
Yes. Turn on “Require survey response” and they must pick a reason. Leave it off and they get a Skip button.
Can I write my own reasons?
Yes. Add, rename, reorder or delete reasons, and give any of them a follow-up question. Seven common reasons are included to start from.
Where do the answers show up?
Downloads → Reports has a Cancellation Survey report with a chart and table. The reason is also shown on each subscription, and everything can be exported to CSV.
Can I use the reason in my cancellation emails?
Yes. The {reason} and {follow_up} template tags work in EDD’s subscription emails.
